CBA is a physician ordered program funded by Medicaid. This program provides home and community based services to eligible aged and disabled adults as a cost-effective alternative to nursing facility care. A DADS caseworker determines the Medicaid eligibility. Our agency works closely with caseworkers to provide and coordinate services following the qualifying criteria outlined by the Department of Aging and Disability Services.

- Skilled nursing care
- Personal care
- Medical supplies
- Adaptive aides
- Minor home modifications
We assist our patients with choices regarding long term care services. Our goal is to provide services to clients by facilitating the development and utilization of services to provide a safe alternative while remaining in the community. |
